There are some conditions that may require a doctor's permission before services can be performed. Here is our downloadable FORM. Have your doctor fill it out and return it before your appointment.
Our Cancellation Policy:
Though we understand that sometimes things happen, because of the size of our business we have a very specific cancellation policy. Should you need to cancel your appointment, we ask that you contact us no later than 6:00PM the night before your appointment, regardless of what time your appointment is scheduled. This gives us enough time to contact our wait-list to fill the slot. If you contact us after 6:00PM the night before or if you do not show up for your appointment, you will be responsible for the full cost of your appointment. If we have a credit card on file, it will be charged. If we do not have a card on file, it will need to be paid before a future appointment can be made. If you are coming in with the intent to use a package, the package will be used as if the guest had arrived. If you prepay for a service and cancel BEFORE 6:00PM the night before your scheduled appointment, the credit will remain on your account to be used at your next visit. If scheduling appointments for two people, a credit card is required. If the group appointments are cancelled within 48 hours of appointment, you will be charged 50% of the service total. If you cancelled within 24 hours or no show, you will be charged 100% no matter the reason. All purchases are final and non-refundable.
